What is a 24-hour Urinary Phosphorus Test?

Phosphorus is a vital mineral, involved in bone health, teeth formation, energy storage, kidney function, cell -membrane function and muscle contraction. It maintains the acid-base balance of the body and is regulated by the kidneys.

The 24-hour urinary phosphorus test measures the amount of phosphorus excreted in urine during the 24-hour period. This test is helpful in assessing kidney function, monitor intake of dietary phosphorus, and metabolic disorders. Abnormal phosphorus levels indicate various disorders including chronic kidney disease, vitamin D imbalances or hyperparathyroidism. Evaluation through 24-hour urinary phosphorus test helps in early diagnosis and treatment.

How is the Test Performed?

The 24-hour urinary phosphorus test involves the collection of urine over 24 hours, this ensures the accurate measurement of phosphorus as levels may fluctuate during the day.
Preparation

  • Patients are advised to maintain their normal diet during the test otherwise instructed by the healthcare provider.
  • Your doctor might recommend to stop taking medications that interfere with phosphorus levels.

Collection

  • Flush the urine first time and note that time (for e.g. 8 am)
  • Collect every drop of urine in the container provided by the laboratory for full 24 hours.
  • Collect the first urine of next morning at the same time.
  • Keep the container in a cool place like a refrigerator or on ice.

Submission

  • Return the sample with proper labelling and seal to the laboratory for analysis

Analysis

The sample will be then analyzed for phosphorus levels using spectrometry or inductively-coupled plasma mass spectrometry (ICP-MS).

What are the symptoms that prompt this test?

A 24-hour urinary phosphorus test is recommended to individuals experiencing symptoms of phosphorus imbalances, kidney or metabolic disorders. Identification of these mineral imbalances is crucial for early diagnosis and treatment of the disease. Here some of the symptoms of phosphorus imbalances are mentioned that prompt this test: or related health conditions.

Symptoms of High Phosphorus

The value of urinary phosphorus excreted through urine vary according to age, sex and diet of the individuals. Generally accepted values are mentioned below;

6-12 years – 130 to 941 mg/24 hrs
13-80 years Male –390-1426 mg/24 hrs
13-80 years Female –261-1078 mg/24 hrs
>80 years Male –210-888 mg/24 hrs
>80 years Female –96-706 mg/24 hrs

**Normal values may vary according to the test assay and individual factors. Consult your doctor for interpretation of the test results…**

What are Normal Ranges for This Test?

Abnormal phosphorus levels indicate various health conditions and is important to diagnose and treat at the earliest;

High Phosphorus Levels Indicate

  • Hyperparathyroidism: Superfluous function of parathyroid glands causes the release of excessive phosphorus.
  • Vitamin D toxicity: Excess vitamin D leads to increased phosphorus absorption from the gut
  • Renal disorders: Renal phosphorus loss leads to bone disorders including osteomalacia and rickets.
  • High Intake of dietary phosphorus.
  • Acidosis

Low Phosphorus Levels Indicate

  • Chronic Kidney disease
  • Severe Malnutrition of phosphorus
  • Hypoparathyroidism
  • Liver disorders

If abnormalities are detected further testing is suggested including blood tests, imaging studies and hormonal tests.

What Happens if Test Results Show Abnormal Levels?

Abnormal levels of 17 OH Progesterone, particularly elevated levels, can indicate Congenital Adrenal Hyperplasia (CAH) or other adrenal disorders. Typically your healthcare provider might demand further testing to confirm diagnosis and assess the severity of your condition. These tests might include genetic testing, imaging studies of the adrenal glands etc.

  • If CAH is diagnosed, treatment often involves hormone replacement therapy to correct the imbalance of cortisol and aldosterone.
  • Ongoing monitoring for individuals diagnosed with CAH or other adrenal gland disorders is necessary to ensure hormonal balance.
