What is a Beta-2 Microglobulin (B2M) Tumor Marker Test?
A Beta-2 Microglobulin (B2M) Tumor Marker Test measures the amount of beta-2 microglobulin (B2M) in the blood and, in some cases, urine. B2M is a protein found on the surface of many cells, particularly immune system cells. Higher B2M levels can occur when there is increased activity or turnover of certain cells. Elevated levels may be associated with some blood cancers, including multiple myeloma, chronic lymphocytic leukemia (CLL), and certain lymphomas.
The test is not used alone to diagnose cancer. Instead, healthcare providers may use B2M levels alongside other laboratory tests and clinical findings to assess disease severity, estimate prognosis, monitor disease progression, and evaluate response to treatment. B2M levels can also increase due to conditions unrelated to cancer, including kidney disease, inflammation, and certain infections. Therefore, an elevated result does not necessarily mean that a person has cancer.
How is the Test Performed?
Preparation
- Fasting is usually not required.
- For CSF analysis, you may be asked to empty your bladder and bowels before the test.
- Inform your healthcare provider about any medications or supplements you are taking.
- Tell your healthcare provider if you have kidney disease or another medical condition that may affect B2M levels.
- Follow any additional instructions provided before specimen collection.
Collection
- For a blood test, a blood sample is collected from a vein in the arm using a sample needle.
- For a urine test, you will be asked to collect a urine sample for the next 24 hours.
- For a cerebrospinal fluid (CSF) analysis, your healthcare provider will perform a procedure called a
- spinal tap, also known as a lumbar puncture.
Lab Analysis
The collected sample is analyzed to measure beta-2 microglobulin (B2M) levels. The results are interpreted alongside the patient’s medical history, symptoms, kidney function, blood counts, and other cancer-related tests. - Frequent infections
- Bone pain

For urine testing, the expected level is generally low because healthy kidneys normally prevent significant amounts of B2M from passing into urine.

What Happens if Test Results Show Abnormal Levels?
