What Is A Bile Acids Test?

Bile Acids test is a diagnostic tool that measures the levels of bile acids in the blood stream. Bile acids are produced by the liver and stored in the gallbladder and play a vital role in digesting fats and absorbing fat-soluble vitamins. The test helps evaluate liver and gallbladder function, detecting conditions such as cholestasis (impaired bile flow), liver disease or bile duct obstruction.

The test is often performed when a patient shows symptoms of liver dysfunction such as jaundice, itching or digestive issues. It is also used to diagnose intrahepatic cholestasis of pregnancy (ICP), a condition that can cause complications for both mother and baby. Additionally bile acids levels may be tested to monitor liver function in patients with known liver disorders or after gallbladder removal.

How is the Test Performed?

Fasting for 8-12 hours may be required for accurate results.

The blood sample is analyzed in the laboratory using enzymatic or chromatographic methods to measure bile acid concentration.

What are the symptoms that prompt this test?

Normal Bile Acid Levels (Fasting)
Men: 0–10 µmol/L
Women: 0–12 µmol/L
Postprandial (after eating) levels are normally higher.
Pregnancy: Levels above 10 µmol/L may indicate cholestasis.

What are Normal Ranges for This Test?

The normal ranges for this hormone can vary based on age, sex and time of the day the test is performed. The general accepted normal ranges are:

Infants – Less than 630 ng/dL (nanograms per deciliter) after birth with levels decreasing after first few days.
Children (pre puberty) – Less than 110 ng/dL for male and less than 100 ng/dL for females.
Adult Women – Less than 285 ng/dL for pre-menopausal women and less than 51 ng/dL for postmenopausal women.
Adult Men – Less than 220 ng/dL for males.

What Happens if Test Results Show Abnormal Levels?

If bile acid levels are elevated, it may indicate liver dysfunction, cholestasis or bile duct blockage. In pregnant women, high levels may point toward intrahepatic cholestasis, which requires immediate monitoring and possibly early delivery to prevent complications.

  • MRI or CT scan of the liver
  • Hepatitis screening
  • Biopsy (in rare cases)
