What is the Cortisol Random?

Cortisol is a steroid hormone produced by the adrenal gland and it has many roles in metabolism, immunity, and stress management. Cortisol is often called so because of the increase or decrease in its levels during stress. Cortisol is involved in the regulation of blood pressure, blood sugar, and inflammation. It is secreted in a diurnal pattern with high levels in the morning and low levels during the day.

A cortisol random test is conducted to measure the levels of cortisol in the blood at any time of the day. Cortisol imbalance may be associated with several other health conditions and can affect the general health of an individual

How is the Test Performed?

A simple blood draw is performed by the process of venipuncture at any time of the day and then analyzed in the laboratory.

Preparation

  • Avoid vigorous exercise before the test
  • Avoid the intake of caffeine before the cortisol random test

  • The sample is analyzed using the techniques of high-performance liquid chromatography-tandem mass spectrometry (HPLC-MS/MS) or immunoassays as they are highly sensitive and accurate.

What are Normal Ranges for This Test?

The normal levels of cortisol fluctuate throughout the day and its results depend on the time of test;

Morning – 6 to 23 mcg/dL
Afternoon – 3 to 15 mcg/dL
Evening – 1 to 10 mcg/dL

What Happens if Test Results Show Abnormal Levels?

Cortical abnormalities may be a sign of underlying adrenal disorders for which further evaluation is needed and management is required. Your healthcare provider may suggest other tests, such as an ACTH stimulation test, dexamethasone suppression test, or imaging of the adrenal glands, based on the results.

High Cortisol Levels

  • Indicative of Cushing’s Syndrome
  • The treatment will depend on the source and can include decreasing the use of steroids, surgical removal of tumors, radiation therapy, or drugs that block the production of cortisol.

Low Cortisol Levels

  • Indicative of Addison’s Disease
  • The standard Regime of treatment is replacement steroid therapy to achieve the optimal level of cortisol. Patients with Addison’s disease may need to take corticosteroid drugs for the rest of their lives.
