What is a PTEN Genetic Test?

A PTEN Genetic Test looks for changes (mutations or variants) in the PTEN gene, which produces a protein that helps regulate cell growth and division. The PTEN protein acts as a tumor suppressor, preventing cells from growing and dividing uncontrollably. Inherited mutations in the PTEN gene are associated with PTEN Hamartoma Tumor Syndrome (PHTS), a group of rare genetic conditions that includes Cowden syndrome. People with these conditions have an increased risk of developing certain cancers, including breast, thyroid, endometrial, kidney, and colorectal cancers.
A PTEN Genetic Test may be recommended for individuals with a strong personal or family history of cancer, multiple noncancerous growths (hamartomas), or physical features suggestive of PTEN-related disorders. The results help guide cancer screening, treatment decisions, and genetic counseling.

How is the Test Performed?

Preparation

  • Preparing for a PTEN Genetic Test is generally straightforward.
  • Fasting is not required.
  • For a CVS or amniocentesis, you may need to drink extra fluids and avoid urinating before the test.
  • Inform your healthcare provider about your personal and family history of cancer or inherited disorders.
  • Discuss the benefits and possible outcomes of genetic testing with a genetic counsellor if recommended.
  • Follow any additional instructions provided before specimen collection.

Collection

  • For solid tumors, a sample of tumor tissue is collected through a biopsy.
  • For blood or bone marrow cancer, a bone marrow test is performed.
  • For a blood test, a blood sample is collected from a vein in the arm using a small needle.
  • For a buccal swab, your healthcare provider will insert a swab inside your mouth to collect a sample of cells.
  • For amniocentesis, a sample of amino fluids surrounding the fetus during pregnancy is collected.
  • For Chorionic villus sampling (CVS), a tissue sample is collected from chorionic villi, a part of the placenta.

Lab Analysis
The collected sample is analyzed using specialized molecular genetic techniques such as DNA sequencing to identify mutations in the PTEN gene. The results are interpreted together with personal medical history, family history, and clinical findings to determine whether an inherited PTEN-related disorder is present and to guide future screening and management.

What are the Symptoms that Prompt this Test?

A PTEN Genetic Test is recommended when a person’s medical history, physical findings, or family history suggests a PTEN-related genetic disorder rather than to investigate a specific symptom.

Symptoms and Situations That May Prompt This Test

  • Large head size (macrocephaly)

  • A strong family history of PTEN-related conditions
  • A known PTEN mutation in a family member

What are the Normal Ranges for This Test?

A PTEN Genetic Test does not have a numerical normal range because it evaluates changes in the PTEN gene rather than measuring a substance in the blood.
Results are generally reported as:

Negative: No disease-causing PTEN mutation was detected.
Positive: A pathogenic or likely pathogenic PTEN mutation was identified.

Variant of Uncertain Significance (VUS): A genetic change was detected, but its clinical significance is currently unknown.

A negative result does not completely exclude hereditary cancer risk because mutations in other genes may also increase cancer susceptibility.

What Happens if Test Results Show Abnormal Findings?

Abnormal PTEN Genetic Test results indicate the presence of a disease-causing mutation in the PTEN gene.

Positive PTEN Mutation
A positive result may be associated with:

  • PTEN Hamartoma Tumor Syndrome (PHTS)
  • Cowden syndrome
  • Increased lifetime risk of breast cancer
  • Increased risk of thyroid cancer
  • Increased risk of endometrial cancer
  • Increased risk of kidney cancer
  • Increased risk of colorectal cancer

Individuals with a confirmed PTEN mutation often require lifelong cancer surveillance and regular health screenings.

Variant of Uncertain Significance (VUS)
A VUS result means:

  • A genetic change was identified, but its effect is not yet fully understood.
  • Further research or family studies may clarify its significance.
  • Clinical management is usually based on personal and family history rather than the VUS alone.

Negative Results
A negative result generally means that no known harmful PTEN mutation was detected. However, hereditary cancer risk may still exist due to:

  • Mutations in other cancer-related genes
  • Family history
  • Other inherited genetic conditions

Depending on the results, your healthcare provider may recommend additional investigations such as TP53 Genetic Testing, BRCA Testing, multigene hereditary cancer panel testing, or genetic counselling.
