What Is A Sickling Test?

The sickling test also known as the sickle cell test, is a diagnostic procedure used to detect the presence of abnormal hemoglobin known as hemoglobin S (HbS) in red blood cells. Hemoglobin S causes red blood cells to become rigid and sickle-shaped, leading to sickle cell disease or sickle cell trait. Sickle cell disease is a genetic disorder that can cause severe pain, anemia and organ damage due to the abnormal shape of red blood cells which can block blood flow. The sickling test is often performed as part of newborn screening programs during pregnancy or in individuals with symptoms of sickle cell disease. It is also used to identify carriers of the sickle cell trait who may not show symptoms but can pass the gene to their children. Early detection through the sickling test allows for timely management and genetic counseling.

How is the Test Performed?

Sample Collection
The test requires a blood sample which is drawn from the arm of the patient.

Lab Analysis
The sample is then mixed with a special solution that reduces oxygen levels, causing hemoglobin S to form sickle-shaped cells if present. The mixture is examined under a microscope to identify any abnormal red blood cells. In some cases, a more advanced test called hemoglobin electrophoresis may be performed to confirm the diagnosis and distinguish between sickle cell trait and sickle cell disease.

What are the symptoms that prompt this test?

Patients of Sickle Cell Disease often show the following symptoms:

Adults and Children

  • Fatigue and weakness
  • Painful swelling in hands and feet (in children)
  • Episodes of severe pain (sickle cell crises)
  • Frequent infections
  • Delayed growth or puberty
  • Vision problems
  • Pale skin or signs of anemia
  • Part of routine newborn screening in many countries
  • Pregnant women may also be tested to assess the risk of passing the sickle cell gene to the baby

What are Normal Ranges for This Test?

Sickling Test results are often interpreted as:

Negative – No sickle cells detected.
Positive –Sickle cells detected indicating sickle cell trait (further testing may be required to confirm)

What Happens if Test Results Show Abnormal Levels?

Further steps depend on the findings:

Positive Result

  • A positive result indicates the presence of hemoglobin S. Further testing such as hemoglobin electrophoresis is required to confirm whether the individual has sickle cell trait or sickle cell disease.
  • Genetic counseling may be recommended for individuals with sickle cell trait to understand the risk of passing the gene to their children.
  • For individuals with sickle cell disease, treatment may include medications, blood transfusions or bone marrow transplants.

Negative Result

No further action is needed if symptoms are absent and there is no family history of sickle cell disease.
