What is CA 15-3?

Cancer Antigen (CA) 15-3 is an essential biomarker in the detection of breast cancer. This protein is produced by the breast cells and is usually elevated in individuals having cancer, especially metastatic breast cancer. The CA 15-3 test is not only elevated when a person is affected with breast cancer antigen but also in cases of ovarian cancer, lung cancer, liver disease, and some non-cancerous conditions including hepatitis and lupus. It is usually used to monitor the effectiveness of the cancer treatment.

The CA 15-3 test is conducted to measure the levels of cancer antigen 15-3 in the bloodstream. This test is not primarily used for the detection of cancers only but is important for monitoring the treatment effectiveness and the recovery of the patient.   

How is the Test Performed?

A simple blood draw is performed for the CA 15-3 test within a clinic or a hospital setting and then analyzed in the laboratory.

Preparation

  • No special instructions are required for the test.
  • Fasting is not typically required for CA 15-3 test.
  • Inform your doctor about any medications you are taking as he might instruct you to avoid the intake of the medicines that might interfere with the test results.


Blood Draw

  • The process is straightforward and requires blood drawn from the vein of the arm.
  • Pressure is applied to the site from where the blood was drawn to stop any bleeding.
  • A bandage is then placed on that site.
  • The sample is then placed in a tube that is chemically treated to prevent the degradation or contamination of the sample.


Laboratory Analysis

  • The sample is sent to the laboratory for the analysis of CA 15-3 in the blood using enzyme-linked immunosorbent assay (ELISA) for accurate results.

CA 15-3 test is suggested by the doctor if an individual has been diagnosed with breast cancer or is showing the signs and symptoms of breast cancer;

Symptoms of Breast Cancer

  • Formation of a lump in the underarm or breast cancer
  • Change in the shape or size of the breast
  • Change in the appearance of the breast
  • Change in the skin of the breast such as redness, dimpling, or scaling
  • Nipple discharge, especially if the blood is discharged from the nipples
  • Pain or tenderness in the breast


Symptoms of Metastatic Breast Cancer

  • Bone pain
  • Headache
  • Dizziness
  • Nausea
  • Vision, speech, or balance changes
  • Weakness and fatigue
  • Difficulty breathing if the cancer has progressed to the lungs
  • If the cancer has advanced to the liver abdominal pain and swelling are observed

What are Normal Ranges for This Test?

CA 15-3 normal ranges vary according to the assay of the test used by different laboratories. The general reference range is given below;

  • Normal – Less than 30 U/mL
  • Slightly elevated – 30 to 50 U/mL (associated with benign and early-stage cancer)
  • Highly elevated – more than 50 U/mL (associated with advanced-stage cancer)


**It is important to consult your doctor for the confirmation of the results as elevated levels are not only associated with breast cancer but can be due to other underlying health conditions. **

What Happens if Test Results Show Abnormal Levels?

Abnormal level of CA 15-3 is not only associated with breast cancer but it warrants additional diagnostic tests for the confirmation of the disease. The common indications are mentioned below;

High CA 15-3 Indicate

  • Breast cancer of advanced-stage or metastatic cancer
  • Ovarian cancer
  • Lung cancer
  • Pancreatic cancer
  • Liver disease
  • Chronic Hepatitis
  • Lupus
  • Inflammatory conditions


Low CA 15-3 Indicate

  • Low levels are not associated with breast cancer.
  • In the early stages of breast cancer CA 15-3 is not elevated and is usually at a low level.

This test is usually not used for the diagnosis of cancer as it does not confirm the presence rather it is used for monitoring the disease.
